vendor: update all dependencies except Azure SDK
The Azure SDK doesn't support Go 1.5 anymore. We can't upgrade it until
Go 1.8 comes out.

all: update license headers to distiguish GPL/LGPL
All code outside of cmd/ is licensed as LGPL. The headers
now reflect this by calling the whole work "the go-ethereum library".
