## Description
### Issue
Some values (like chain ID) were being leaked from the previous block/initialization into PrepareProposal and ProcessProposal, these values are only available if:
1. The node has never been stopped since the genesis block (as these values are set on `InitChain`)
2. The node has already commited a block (as the previous header was being used for the new state of prepare and process proposal).
So if a node is restarted, during the first prepare and process proposal these values won't be populated, and that will cause issues if they are being used.
### Solution
Remove any previous header information from a previous block in the prepare and process proposal contexts, making things consistent at every height.
- Added ChainID to baseapp
- Use an empty header in Commit() with only the chain id set
- Fix context for prepare and process proposal

## Description
removes the dependency of tendermint/utils/log from countless locations. This is in effort of reducing Tendermint's lib usage in the sdk
this is nonbreaking as the interface is the same. To eliminate tm/utils/log in the sdk we need a few more things. Once we have fully removed the tendermint logger, I would propose we break the interface and define our own for our use case, when we pass the logger to tendermint in the node.New() function we can wrap our logger for its use case

Removes `simapp` as a dependency from `testutil/network` by abstracting `simapp` construction behind a factory fn (now in `simapp`), then requiring callers of `network.DefaultConfig` to pass in the factory fn directly.
This patch is an intermediate step to removing all dependencies on `simapp`. Follow up work may include moving all of the tests using `network.DefaultConfig` updated here up to `simapp`, or possibly just the runners.

## Description
When locally working with golangci-lint, we can see that there were many deprecation warnings about sdk.Int. This PR resolves that and makes 1-2 other linting related changes. Issue on linting coming next.
This also moves BitCurve to bitCurve.
I expect that this set of changes will require several pull requests, one of them to the settings for the linter.
It also does a gofumpt, because we had various formatting-related linters fail, too.

### Root cause
Prior to merge of #11924, SimApp calls [module/NewManager](2b549b807c/simapp/app.go (L344)), which implicitly sets sane default values of [OrderExportGenesis](2b549b807c/types/module/module.go (L239)) from the modules passed as parameters. Since this usage was deprecated in favor of [RegisterModules](823d2a7f28/simapp/app.go (L354)) in the app wiring work, `OrderExportGenesis` was now `nil`, resulting in empty `genesisData` in [ExportGenesis](823d2a7f28/types/module/module.go (L339))
### Fix
Explicitly set `OrderExportGenesis` with same module order as `OrderInitGenesis` from `SimApp`.

We decided to remove middlewares, and revert to antehandlers (exactly like in v045) for this release. A better middleware solution will be implemented after v046.
